Inner Healing Liability Waiver Agreement *

I understand that Revive Church Inner Healing Ministries will be administering Inner Healing prayer to give spiritual and emotional help in healing prayer methods and may or may not be clinically demonstrated. I fully understand that all Revive Church Healing Ministries are under the control of the Holy Spirit and no guarantees are made. I fully understand that the ministry I receive is not counseling in any form but instead, prayer ministry. Thus, I waive all rights to claims of any and all liability whatsoever. I accept that they may recommend further ministry for me by a Pastor, licensed counselor, therapist or another agency within or outside of the Revive Church community.

I acknowledge that I understand I am free to terminate my participation at any time for any reason. I also acknowledge and understand that I am fully responsible for any and all consequences and fully responsible for the choices involved in my post healing and/or any suggested follow up sessions of inner healing, counseling or otherwise. I agree to the expectations of my commitment with Revive Church Inner Healing Ministries.
